Overview
The best way to choose a gift for your friend is to consider his personality and the type of friendship you have. Is he confident and able to laugh at himself? Do you have the kind of relationship where you make fun of each other and play practical jokes? If so, a gift like this could be perfect. Or if not, should you just get him something nice and normal?
How to decide if a silly or offensive gift is appropriate
As I mentioned above, the main things to consider are what your friend is like, and what your relationship is like.  It's also worth considering whether you want it to be a 'naughty' or 'offensive' gift.  He might be ok with one but not the other.
Let's consider both of those things in more detail:
What sort of person is he?  Is he timid and reserved, or bold and confident?  Another thing to think about here is whether he is likely to open the present in front of his friends (for example at a party) or at home with his family.  If it's likely to be in front of friends then the odds are that he'll love it (and even if he doesn't he'll pretend that he does).  If it's at home with his mum then maybe not.
What sort of relationship do you have with him? This is probably more important than the sort of person he is.
